NEW PATIENT INFORMATION
We are honored that you have chosen our office for Orthopaedic
Care.  So we can quickly do the necessary paperwork, please
do the following prior to arriving in the office:
1)
 Fill out the Registration Form and History Form.  
Note you can either use the regular format or PDF
format.  The PDF format <should> allow you to mostly
fill in the form in your browser.   PRINT them out, sign
and date in the appropriate places and bring them to
the office with you.  Be sure to bring your insurance
information (and referrals if necessary)
2)
 Read the Patient's Rights Form
3)
 Print out and sign the HIPAA Disclosure Form.
4)
 Bring all these forms with you to the office
